When should handling of extremely prematurely born infants, those born at the border of viability, focus on survival, and when should it focus on comfort-care? This is one of the most agonising choices facing healthcare, parents, and society. In this opinion, the Swedish National Council on Medical Ethics offers ethical guidance on the proper management of infants born after 22 to 23 weeks of gestation, including the roles and rights of the parents in the crucial decision-making process.
